Summary of Results.
\nDuring 2022\/23 season, samples collected from influenza-like-illness patients as of April 20, 2023 were as below:<\/p>\n

From main island of Japan and Okinawa 199 samples were collected at the first visit, 61 samples at the second visit, 40 samples after baloxavir treatment (Table 1).<\/p>\n

The results of the detection of PA\/I38T substation by cycling probe real-time PCR are shown below (Table 2).<\/p>\n

The percentage of patients with PA\/I38T substitution was:
\nIn A(H1N1)pdm09, no PA\/I38T variant in pre- and post-treatment samples was detected out of 1 sample (0.0%). In A(H3N2), among pre-treatment samples 1 PA\/I38T variant (0.6%) was detected out of 176 samples. Among post baloxavir-treatment samples, 3 PA\/I38T variants (7.9%) were detected out of 38 samples. If the frequency is calculated based on RT-PCR-positives at the second visit, it became 17.6% (3 PA\/I38T variants out of 17 RT-PCR-positive samples at the second visit).<\/p>\n

In unsubtyped influenza A, pre-treatment samples were 0 positive (0%) PA\/I38T variant out of 11 specimens.<\/p>\n

Influenza B was not detected.<\/p>\n

[PA gene analysis]<\/p>\n

In A(H3N2), one PA\/I38M, and two I38T substitutions (13.0%) out of 23 paired baloxavir-treated clinical sample was detected by next-generation sequencing (Table 3). If the denominator is based on 7 samples with successful NGS in the baloxavir-treated group, the frequency of PA variant was 42.8%.<\/p>\n

No substitution in NA gene which confer reduced sensitivity to NAIs was found by next-generation sequencing either in pre-treatment samples (0\/103) or post-oseltamivir-treated samples (0\/13) (Table 4).<\/p>\n
